Operation Eyesight Canada has been a Registered Charity in Canada since 1967 (No. 119068955 RR0001) 
and operates under the name and style of Operation Eyesight Universal.

These are the Donation Payment Terms and Conditions which govern your use of the donation payment 
services provided through the web domain www.operationeyesight.com (the “Website”).

By using our payment submission process, you agree to be bound by these Terms and Conditions 
applicable to your donation, as well as the Website Terms & Conditions of use as further identified below.

In these terms and conditions, “We/us/our” means Operation Eyesight Canada “You/your” means you as 
a user of the Website.

We will use your donation at our discretion but within our stated charitable objectives.

All payments through the Website are to be made by credit card. Payments may include one-time donations, 
or regular (monthly) payments. When you select monthly payments, you agree that you are scheduling a 
series of donations to be made on the day of the month, consistent with your original payment date, until 
further notice or cancellation. You agree that if you make a series of donations through our Website, each 
of these donations is covered by these Terms & Conditions, and you authorize our payment service provider 
to request funds from your credit or debit card on the same day each month on a recurring basis until 
these services are cancelled.

To complete a donation through the Website, we will require certain information from you, including your 
name, billing address, email address, telephone number, and credit card details in order to process the 
payment.

When you submit your payment details, this information will be transferred to our payment provider, and 
your data will be collected and processed securely by them. Please visit our Privacy Policy for more 
information on how we maintain and protect your data.

We do not provide refunds of donations except under limited circumstances that are deemed appropriate 
at our sole discretion.

These exceptions and circumstances may include, but are not limited to:

If a refund is issued, we will cancel the original donation receipt issued to the donor.

Unless otherwise agreed by us in our sole discretion, refunds are subject to a 30-day waiting period so 
that we can confirm the legitimacy and receipt of the original donation payment prior to providing the 
refund.

We reserve the right to amend these Donation Payment Terms & Conditions at any time. Each time that 
you access the Website or submit a Donation Payment through our online services, you agree to be subject

This Agreement and any action related thereto shall be governed by the laws of the Province of Alberta. 
The parties consent to the exclusive jurisdiction of the courts of Alberta, Canada in resolving any dispute 
which may arise in relation to a donation or a donation refund.

If any provision of this Agreement, or any portion thereof, is held to be invalid and unenforceable, then the 
remainder of this Agreement shall nevertheless remain in full force and effect, and the invalid or unenforceable

of the original provision.
